Office of the Student Formation
The Office of the Student Formation (OSF) is under the Office of Student Development and Well-Being (OSDW). OSF is in charge of the enforcement of student discipline policies, rules and regulations as stipulated in the student handbook. The Office is headed by the Coordinator who is directly supervised by the Director of SDW. His/her specific functions include:
- handling all cases of minor and major offenses;
- facilitating the hearing of major offenses, recommending courses of action to the Director of SDW and implementing such course of action;
- creating the Peace Committee who shall hear all cases of major offenses when deemed necessary upon the approval of the VPE thru the recommendation of the Director of SDW;
- supporting and performing Administrative functions of Peace/Grievance Committee;
- creating and selecting members of the ad hoc peace committee;
- receiving all referrals of minor and major offenses, maintaining and recording all offenses and violations of students;
- promoting student formation on discipline to maintain proper student’s behavior and decorum whether inside or outside the campus;
- ensuring peace and order during students’ institutional activities;
- conducing investigation and necessary proceedings in dealing with all minor and major student offenses;
- enforcing corrective measures on students who violate school rules and policies in coordination with the Director for Student Development and Well-Being/Student Affairs and Services;
- maintaining order and discipline in the campus by implementing school rules and regulations based on the student manual;
- submitting a summary report for the Student Development and Well-Being regarding the Ad-hoc Peace Committee recommendations on courses of action for cases of major offenses heard by them;
- coordinating with the Coordinator for Physical Plant, Safety and Security on student’s concerns regarding uniform, invited guests, campus curfew, food entry etc.;
- performing other related functions as may be delegated by the Director for Student Development and Well-Being/Student Affairs and Services.
Peace Committee
The SJDEFI Peace Committee may convene to hear, deliberate and decide on appeals submitted by students and faculty and on cases referred by the Deans/Coordinator.
Composition of Peace Committee
The Peace Committee shall be headed by the Coordinator for Student Formation, and shall be composed of the following individuals:
- Coordinator for Student Formation as Chair;
- Respective Dean/Program/Department Coordinator as Co- chair;
- SJDEFI Faculty Association President or his/her authorized representative;
- SJDEFI Campus Minister or RE Faculty;
- Current SCB President or his/her authorized representative
- A member/or members as determined by the Coordinator for Student Formation
Functions of Peace Committee
- The Committee shall review, conduct formal investigation and hear cases of major offense.
- The Committee shall be fact-finding and render a decision based on the set provisions of the handbook.
- The Committee shall agree on its ground rules and procedures for hearing of complaints based on due process.
c1. The Committee shall render a decision within 15 days from the date of last hearing. The decision should be in writing and shall state the reasons for such decision and must be submitted to the Office of Student Discipline Formation. The Coordinator for Student Formation shall make a recommendation out of the committee’s decision and shall present such recommendation to the OSDW and the President/VPE for approval.
c2. Any party not satisfied with the decision rendered may ask for a reconsideration with the Office Student Discipline Formation stating the reasons within ten days from the receipt of the decision. The decision of the Committee, upon the approval of the President/VPE shall immediately become final and executory if neither party avails of this remedy within the prescribed period.
Uniform Code and Guidelines
Official School Uniform
All students are required to wear the prescribed uniform with I.D. at all times within the school premises, except on special occasions like:
- departmental or institutional activities if other dress code is required; and
- scheduled activities during Saturdays or non-class day. In such cases, the student should be able to present a letter of request for not wearing the prescribed uniform, noted by the Dean or organizer of the activities and approved by the Coordinator of Student Formation. Triplicate copies of approved letters of request will be submitted. Copies shall be provided to the Safety and Security Office, Office of Student Formation and the respective Dean.
Prescribed Uniform
All students are required to wear the prescribed uniform from Monday to Friday as follows;
Male Student Uniform
- White polo, black pants, black leather shoes with black socks;
- Haircut should not be touching the base of the ear and the collar of his polo, and bangs should not cover his eyes/eyebrows;
- White, plain, round neck shirt or sando undergarments while wearing the prescribed uniform; and
- At no time shall the male students refrain from wearing their polo shirts while inside the school premises. However, in cases of laboratory classes, a plain shirt may be worn provided they wear their laboratory gowns.
Female Student Uniform
- White blouse and skirt with necktie and black shoes with at least one-inch but not more than 2-inches heels. The length of the female student’s skirt should be at least an inch below the knee. No miniskirts are allowed;
- Foot socks may be worn provided they are skin tone or black and at least 1 inch below the ankle;
- Sandals with straps and open toed-shoes are not allowed; and
- White undergarments or white sando.
Physical Student (PE) Uniform
Students are also required to wear other types of uniforms such as Physical Education (PE) uniforms. PE uniform consists of dark blue jogging pants, white t-shirt with SJDEFI seal and rubber shoes with socks.
PE uniforms shall be worn only during the scheduled PE class. However, if the PE class falls to be the first class, students may enter wearing it with their IDs. Students should wear their prescribed classroom uniforms if an academic subject is scheduled after their PE class. However, students could ask for a request to wear PE uniform provided a letter of request will be submitted by the class president, noted by the subject teacher, and approved by the dean.
Prohibited inside the school premises
For Female Students
- Backless, spaghetti straps, see-through, hanging and low or plunging neckline blouses and shirts with indecent prints;
- See-through skirts or dresses, miniskirts;
- Pants which are dirty, with holes (ripped jeans) or those with indecent prints;
- Skimpy shorts, leggings, and croptop;
- Apparel bearing other University’s or College’s name;
- Cross-dressing;
- Earrings that are dangling or larger than 25-centavo coin, fashionable, more than one pair and those that are inappropriate while wearing the school uniform; and
- Loud-colored and highlighted hair.
For Male Students
- T-shirts which are dirty, with holes or with indecent prints;
- Earrings;
- Loud colored and highlighted hair;
- Long hair, fashionable or trendy indecent hair cut styles cut;
- Cross dressing;
- Sandals, slippers, rubber shoes while wearing the prescribed uniform; and
- Apparel bearing other University’s or College’s name.
School Identification Card (ID)
- Students who are officially enrolled must have their school ID validated every semester upon enrollment. The school ID must be presented to the Security Guard upon entry into the campus and/or to school authorities when asked to do so. School ID should be worn at all times inside the premises of the College. It serves as the students’ access to the school campus.

The school ID must be worn conspicuously with the prescribed official school ID lace inside the campus. Lost ID must be reported immediately to the proper authorities. In cases where the student’s ID card has been lost for any reason, he/she should do the following:
b.1 Report the lost ID card immediately to the Office of Student Formation, get a “Temporary Campus Entry Slip” which shall be valid only for one week or more when necessary so that a temporary I.D. will be issued to him/her; and
b.2 Apply for a new school ID with an affidavit of loss and present receipt of payment for its processing to the Office of the Student Formation. Only upon meeting these requirements shall the Office of the Management Information System process a new ID.
