Admission Procedure

1. The student applicant fills out the form from the website.
• Senior High School
• College
2. The student applicant will receive a message of confirmation from the Admission Office within 5 working days.
3. The student applicant will choose their preferred date for entrance examination and interview.
4. The Admission Office will send a letter of notification within 5 working days regarding the applicant’s status.
5. A non-refundable reservation of slot is required amounting to:
   • Php. 1,000.00 (College)
   • Php. 500.00 (SHS voucher recipient)
   • Php. 1,000.00 (SHS non-voucher recipient)
   • The said amount is deductible from the total first semester tuition fee due upon enrollment. Payment can be done directly to the Finance Office or through online bank deposit.
6. The applicant waits for the schedule of online enrollment to be announced by the Admission Office.

1. Photocopy of latest report card (photocopy of Gr 10 latest semester or Gr 9 Report Card) ID (2x2) Photo – White background

College

1. Photocopy of latest report card (photocopy of Gr 12 latest semester or Gr 11 report card)
2. ID (2x2) photo
• BS Nursing – yellow
• BS Medical Technology – green
• BS Physical Therapy – blue
• BS Hospitality Management – red
• BS Office Administration – orange
• BS Entrepreneurship – orange

Transferees and Second Courses

1. Transcript of records or certification of grades from previous school – for evaluation purposes
2. ID Photo as stated above

Enrollment Requirements

Senior High School

1. Gr 10 original copy of report card (Form 138)
2. Photocopy of PSA/NSO birth certificate (bring original for comparison)
3. 2 pcs ID (2x2) Photo – White background

College

1. Gr 12 original copy of report card (Form 138)
2. Photocopy of PSA/NSO birth certificate (bring original for comparison)
3. Good Moral Certification (original copy)
4. 2 pcs ID (2x2) photo
• BS Nursing – yellow

For College Transferees

1. Certification of Ranking with school seal, specifying the total number of graduates

For Foreign Students
